1. 5. A participant in the “Ready for Labor and Defense” complex is obliged to carefully store the registration card. The participant reports the loss of the registration card to the Testing Center.

6. The issuance of a duplicate of the Complex’s registration card is made by decision of the head of the Testing Center. On the title page of the duplicate the inscription “DUPLICATE” is written. All data about the Complex participant for the entire period is entered into a duplicate of the complex’s registration card based on information stored in the automated information system of the complex and the original test reports of the Complex, which are stored in the Testing Center.

1 Appendix 1 REGULATIONS ABOUT THE RECORD CARD FOR COMPLETING STATE REQUIREMENTS of the All-Russian Physical Culture and Sports Complex “Ready for Labor and Defense” (GTO) Project I. General provisions 1.1. The registration card for fulfilling the state requirements of the All-Russian physical culture and sports complex “Ready for Labor and Defense” (GTO) (hereinafter referred to as the Registration Card) was developed in pursuance of paragraph 18 of the Regulations on the All-Russian physical culture and sports complex “Ready for Labor and Defense” (GTO), approved by the resolution Governments Russian Federation dated June 11, 2014 540, and paragraph 9 of the Procedure for organizing and conducting testing of the population within the framework of the All-Russian physical culture and sports complex “Ready for Labor and Defense” (GTO), approved by order of the Ministry of Sports of the Russian Federation from The registration card was developed for the purpose of entering and storing information about participant and the results of his tests of the GTO complex. 1.3 The registration card is a paper document in which the personal data of the participant, the final results of the tests of the RLD complex, medical clearance marks, information about the issued insignia and the assignment of sports ranks of the RLD Complex are noted. In accordance with clause 9 of the Procedure for organizing and conducting testing the population within the framework of the TRP Complex (hereinafter referred to as the Testing Procedure), the following data is entered into the Registration Card: - ID (individual identification number) of the participant received upon first registration in the Automated Information System “TRP Complex” (hereinafter referred to as AIS TRP); - photograph of the participant measuring 3x4 cm; - last name, first name, patronymic (if available); - floor; 1

2 - date of birth; - a note about the presence of a medical report; - stage of the complex structure in which the participant is tested; - types of tests (tests) that the participant performs in accordance with state requirements for the corresponding stage of the complex structure; - results of tests (tests), certified by the signature of a sports judge; - information about awarding the complex insignia; - availability information sports category(if available) The registration card duplicates information about the results of the participant's performance of tests of the GTO complex, which are entered into the Test Report of the GTO complex. The registration card is issued to the participant once upon the first application to the Testing Center to perform tests and is permanent. The registration card is filled out by the following employees of the Testing Center: - the administrator of the Testing Center regarding personal data upon first contacting the Testing Center; - sports judges in terms of entering the results of tests (tests). If a participant loses the Registration Card, the Testing Center issues a duplicate to the participant. Repeated issuance of a duplicate is not provided. The registration card is kept by the participant of the GTO complex. II. Technical description registration card 2.1. General description. The index card is a soft-cover book measuring 300 x 105 mm, folded in half. The cover is made of coated paper with a density of 170 g/m2, the glossy lamination of the cover is 35 microns, the internal printing block is made of offset paper with a density of 80 g/m2. Outer side the cover contains the title “Registration card for fulfilling state requirements of the GTO complex”, as well as full-color 2

3 elements of the corporate identity of the GTO complex, applied by offset printing. The reverse side of the cover remains blank, without any information added. The pages are fastened together with metal staples along the scoring line. The content of the Registration Card includes several blocks, each of which corresponds to one of the stages of testing specified in the Testing Procedure Personal information about the participant. The first page contains the personal data of the owner of the Registration Card, as well as information about affiliation with the Testing Center and a unique, individual ID number of the participant in the GTO complex. This information is also used to establish the identity (ID) of the participant and enter data on the results into the Protocol for fulfilling the state requirements of the GTO complex when presenting the Registration Card to the sports judge on the site during the tests. The unique identification number is a digital code that organizes the registration of citizens of the Russian Federation who are participants in the GTO complex. 3

4 A unique individual number (ID) is a three-element coded numeric designator. The first element consists of a two-digit number of the last 2 digits of the year in which the participant completed initial registration at the Testing Center. The second element is a two-digit code of the subject of the Russian Federation. The third element is a seven-digit number, which is the serial number of registration in the AIS “GTO” within the constituent entity of the Russian Federation. A mark on the availability of medical clearance. The second spread of the Registration Card contains lines for making notes on the participant’s medical clearance to comply with the standards of the RLD complex, which are prerequisite. The medical clearance note is entered into the Registration Card by an employee of the medical institution at the place of examination and certified with a signature and seal in accordance with the instructions agreed upon by the Ministry of Health of the Russian Federation. 4

Information on compliance with standards: Pages 6 to 25 of the Registration Card include a form for entering information about the types and results of tests performed by a participant in the GTO complex: 1) level of the GTO complex in accordance with the age of the participant; 2) the name of the test is filled in by the sports judge upon presentation of the Registration Card after completing the test; 3) result - best result(if there are several attempts), recorded by the sports judge when the participant performs the test; 4) date of passing the test; 5) Full name of the sports judge responsible for recording the results when testing the GTO complex; 6) signature of the sports judge - the handwritten signature of the sports judge responsible for recording the results when performing tests of the RLD complex, confirming the accuracy of the result entered by him; 7) assessment of the compliance of the result with the insignia for each type of test; 8) conclusion (based on the results of all tests corresponding to the age level) on the compliance of the results with a bronze, silver or gold insignia. 5

6 If the results do not correspond to one of the types of insignia of the GTO complex, a dash is placed. The conclusion is certified by the signature and seal of the responsible employee of the Testing Center Information on awarding insignia of the GTO complex. Page 26 contains a form for entering information about awarding insignia of the GTO complex. This spread contains columns for entering information about the type of insignia (gold, silver, bronze), the number of the level at which it was awarded, the date of issue and information about the document on the basis of which the participant is awarded the insignia of the GTO complex. Data in the specified form is entered by the administrator of the Testing Center at the request of a participant of the GTO complex upon presentation of a certificate issued for the insignia of the GTO complex Information on the availability and assignment of sports categories 6

7 On the page of the Registration Card there is a form for entering information about the existing and (or) sports categories of the GTO complex or other sports categories in sports assigned to the participant. The information includes information about the rank number, the sports discipline for which the rank was assigned, and information about the document on the basis of which the sports rank was assigned. Reminder of the Registration Card Owner. 8 Appendix 3 EXPLANATORY NOTE to the draft Regulations on the registration card for fulfilling state requirements of the All-Russian Physical Culture and Sports Complex “Ready for Labor and Defense” (GTO) In order to organize accounting of citizens’ compliance with the state requirements of the All-Russian Physical Culture and Sports Complex GTO (hereinafter referred to as the GTO complex) ANO The “Executive Directorate of Sports Projects” has developed this draft Regulation on the registration card for fulfilling state requirements of the GTO complex (hereinafter referred to as the Registration Card). When forming this draft Regulation, the following forms and types of record books (data cards) were taken as a basis: qualification record book of first-class athletes, candidates for master of sports, masters of sports and masters of sports of international class (approved by the Order of Rossport); test qualification book for athletes of the second, third categories and youth categories (approved by the Order of Rossport); test book for students mastering secondary education programs vocational education(approved by Order of the Ministry of Education and Science of the Russian Federation dated April 5, 2013 No. 240); test book for students mastering bachelor's degree programs, specialty programs, master's programs (approved by Order of the Ministry of Education and Science of the Russian Federation dated March 22, 2013 203). Additionally, the following legal acts were taken into account: - Regulations “On Population Testing Centers for performing types of tests (tests), standards, requirements for assessing the level of knowledge and skills in the field of physical culture and sports and the procedure for their creation” (approved by the Order of the Ministry of Sports of Russia dated g. 954/1); - The procedure for organizing and conducting testing of the population within the framework of the All-Russian physical culture and sports complex “Ready for Labor and Defense” (approved by Order of the Ministry of Sports of Russia dated 739), 8

9 - Regulations on the All-Russian Physical Culture and Sports Complex “Ready for Labor and Defense” (approved by Decree of the Government of the Russian Federation dated 540). The proposed layout of the Registration Card contains the optimal number of pages, the ratio of rows and columns for entering and storing information about the progress of testing by a participant in the GTO complex, based on the Procedure for organizing and conducting testing of the population. The first page is relevant not only from the point of view of formal personalization of the Registration Card, but also allows responsible persons (administrators and sports judges of the Testing Centers) to identify the participant when presenting it when appearing at the Testing Center. The second spread of the Registration Card for making notes on the medical report allows you to make no more than 20 notes. 20 marks correspond to the maximum possible number of marks on medical clearance to perform tests of the RLD complex for the period of participation in the complex from stages I to VI (up to 24 years inclusive), taking into account the validity of a medical permit for a period of 1 year. In this case, the maximum number of attempts to perform tests is equal to and the second line is a spare one. The number of pages for entering information about the types and results of tests performed by a participant in the GTO complex is determined by the calculation of the maximum permissible quantity possible attempts to test the GTO complex. Thus, participation in the GTO complex from stages I to VI (up to 24 years inclusive) covers the age period from 6 to 24 years, which, provided that the standards are met once during school year(September-May) allows you to fulfill the standards no more than 19 times. Form 20 for entering information about the types and results of tests is a spare one. Number of lines for making entries about issued insignia and entries about existing and (or) sports categories assigned to a participant 9

10 of the GTO complex corresponds to the maximum number of signs possible for issuance at stages I-VI of the complex and corresponds to the numerical value of 20 for each type of record. Thus, the proposed layout of the Registration Card is optimal and has a number of significant advantages over alternative approaches that involve producing a Registration Card for each stage of the GTO complex separately. The use of this layout of the Registration Card will allow: to ensure the storage of data about the participant of the GTO complex for at least half of the period of his participation in the events of the GTO complex, to observe the dynamics of the test results of the GTO complex for at least 30 years; significantly reduce the cost of circulation due to unification and the absence of the need to produce 11 different types of Account Cards; to increase the mobility of participants in the GTO complex in terms of using this version of the Registration Card when passing tests in different Centers when changing their place of residence; start maintaining a Registration Card from any level, while starting tests of the GTO complex from stages V or VI will allow you to use only one Registration Card for the entire period of participation in the GTO complex. 10

What is the GTO complex?

The all-Russian movement “Ready for Labor and Defense” is a physical training program that existed in our country from 1931 to 1991, which covered the population aged 10 to 60 years. Types of standards and tests (tests) included in the VFSK GTO are aimed at determining the level of development physical qualities human: endurance, strength, flexibility and speed capabilities.
